Chronic disease management: asthma and diabetes
There have been three quite distinct health promotion schemes since the introduction of the 1990 contract. The first was based on health promotion clinics, the second on an administratively complex system of measuring health promotion activities focused on the prevention of coronary heart disease and stroke, and the present one which is a greatly simplified, more flexible and professionally led scheme. The arrangements for chronic disease management have continued throughout these changes virtually unaltered.
