The new out-of-hours arrangements have required important changes to both the terms of service and the Red Book.
The terms of service have been amended to emphasize that GPs themselves are responsible for judging whether a consultation is required, when it should take place and where - at the surgery, some other clinical setting such as a primary care centre, or the patient's home - or whether direct referral to hospital is required.
A further amendment allows GPs to transfer their out-of-hours responsibility, in full or in part, to another GP on a health authority list.
